That is the question. No, toner is not necessary, but if you enjoy it, by all means, add that step to your regimen. I am a fan of finding a regimen that works for you, so if toning makes you happy, stick with it! (I love multi-step regimens, but then again, I am a dermatologist.)

In general, toners are used after cleansing to remove excess oils and residual makeup, but they are also used to restore the pH balance of the skin and to prep for the skin for the steps that follow. Toners also can be a great way to gently and easily cleanse in the morning for the more low-maintenance type.

There are many kinds of toners on the market that do different things and it can be confusing. Here’s the lowdown:

  • Some have a nice mix of exfoliators to help remove dead skin cells and allow your other products to work more effectively. One of my personal favorites is our June Product of the Month: Rejuvenative Toner by SkinMedica. Not only does it smell absolutely wonderful, but it has 7% AHA (alpha-hydroxy acids) including lactic, glycolic, malic acids, green tea extract, vitamin A and salicylic acid.
  • Toners can be very useful for people with oily skin. You want to look for a product with salicylic acid, which can help wick away excess oil and calm acne breakouts—a good one to try is Glytone’s Acne Cleansing Toner.
  • Hydrating toners, on the other hand, are probably not necessary if you are using a good moisturizer, but they can be very refreshing and many have soothing properties. One of my favorites is Murad Hydrating Toner, which may be a good choice for someone in their 40s or 50s with drier skin due to hormonal changes.
  • A couple of years ago, I gave one of my good friends Murad’s Essential-C Toner—and of all the things I have given her through the years, she absolutely loves this and feels like it changed her skin like no other product. So, I guess for some, toners are a must!

Two Great Options for Acne

LED Light Therapy

PDT uses a solution called levulan to further activate P. acnes (the bacteria associated with acne) and also targets the sebaceous (oil) gland. Levulan is applied to the face one hour prior to the treatment (you can check email, surf the web, or watch a movie/show on an iPad). After the solution is rinsed, you sit under the blue light for a few minutes.

PDT Facts:

  • Strict sun avoidance for 24-48 hours is necessary following PDT.
  • May have some post-treatment redness, warmth, tingling.
  • A great option for chest and back acne.
  • A good treatment for the reduction of pores, oily skin, and overall skin rejuvenation.

We recommend 3 treatments—once a month for 3 months. The cost is $350 per treatment.

If you’re a parent, you have probably seen molluscum contagiosum. Most of our kids have had them at one time or another. Molluscum are shiny, umbilicated (having a little central dell) bumps that are common in kids. Often confused with warts, MC is caused by a virus that is usually passed from skin-to-skin contact. They’re not dangerous and if left alone, they may go away with time. However, we like to treat them so that they don’t increase in number or spread to others.

We usually treat molluscum using cantharidin, the extract of the blister beetle commonly referred to as “beetle juice”—kids love the name and they also love that it is painless. We drop a little bit on each lesion and hope to get a mild reaction (maybe even a blister). Over the next few days to weeks, the lesions may get a little irritated and hopefully fade away. We usually treat them at one month intervals until they are gone.

Organic Skin Care

Natural and organic skin care products are tough and can be confusing for consumers. Even though the label may say organic, you really need to look for the organic seal, which actually differs from a product that is 100% natural. There are strict guidelines for a product to truly meet “organic” criteria, and few product lines do this.

While it’s a wonderful idea in theory, I have a hard time finding a product or product line that meets natural or organic criteria that is as effective in the anti-aging arena as their non-organic or non-natural counterparts. I feel like if you are going to invest in skin care, USDA Certified Organic Sealyou should choose products that are going to give you the best results for your particular skin type—it’s hard to do with the limited amount of natural or organic products on the market today. Also, most of the natural or organic products contain a lot of essential oils and botanical ingredients, which can cause irritation and allergy. Just because a product is organic, doesn’t necessarily mean that it is better for your skin.

That being said, I think the Suki line, which is 100% natural, does a great job. The Éminence line is a popular certified organic line with a wide array of excellent products—we carry the Yam & Pumpkin Enzyme Peel and love it. However, if you really want something that is going to fight wrinkles, fight sun damage, and help you turn back the clock, I would stick with a line that uses the agents proven time and time again to be efficacious—even if they contain synthetic ingredients.

Basal Cell Carcinomas

Five Warning Signs of Basal Cell Carcinoma

Basal cell carcinomas (or BCCs) are the most common form of skin cancer and something that we see on a daily basis in our clinic. They usually present as a little pimple or sore that doesn’t seem to go away or a persistent scaly spot. We see a lot on the nose, but others areas of the face are commonly affected—as are the ears, neck and scalp. I have been seeing a lot on women’s chests lately (remember those days of frying our chests?). Men’s backs are also a common area. These skin cancers are not life threatening and most can be treated easily in the clinic but it’s important to catch them early. If let go, they can become locally aggressive or could very rarely spread to other parts of the body. Sun protection is the best way to prevent them.

The Skin Cancer Foundation has outlined the five warning signs of basal cell carcinomas:

  1. An open sore that bleeds, oozes, or crusts and remains open for a few weeks, only to heal up and then bleed again. A persistent, non­-healing sore is a very common sign of an early BCC.
  2. A reddish patch or irritated area, frequently occurring on the face, chest, shoulders, arms, or legs. Sometimes the patch crusts, and it may also itch. At other times, it persists with no noticeable discomfort.
  3. A shiny bump or nodule that is pearly or translucent and is often pink, red, or white. The bump can also be tan, black, or brown, especially in dark-haired people, and can be confused with a mole.
  4.  A pink growth with a slightly elevated rolled border and a crusted indentation in the center. As the growth slowly enlarges, tiny blood vessels may develop on the surface.
  5. A scar-like area that is white, yellow or waxy, and often has poorly defined borders; the skin itself appears shiny and taut. This warning sign may indicate the presence of an invasive BCC that is larger than it appears to be on the surface.

Eye Got You Covered!

Why am I getting fine lines & wrinkles around my eyes?

One of the most common questions we get! Fine lines and wrinkles around the eyes are particularly common in both men and women. The onset of wrinkles in this area can occur considerably sooner than elsewhere on the face. The increased frequency of movement around the eyes—due to smiling, squinting and other facial expressions—allows for expression lines to become etched-in more quickly than the rest of the face. That, and the fact that the skin around the eyes is very thin, eye wrinkles can appear at almost any age. They will also appear more quickly if a person has a substantial amount of sun damage (sun worshippers, beware!). While there is no surefire way to guarantee you will never wrinkle, there are steps you can take to help prevent (and treat) eye wrinkles.

How to approach lines around the eyes?

Like most treatments in dermatology, I believe a multi-faceted approach will allow for the best outcome. I recommend tackling lines around the eyes in several ways. (Dark circles are a whole other topic, but these tips help those too!)


SkinMedica Daily Physical Defense SPF 30+It is important to be diligent about sunscreen use to prevent further damage. Sun damage is the leading cause of fine lines and premature aging. In fact, many experts agree that wrinkles are the direct cumulative result of decades of unprotected sun exposure, rather than aging itself. Ultraviolet (UV) rays penetrate the skin, invading the normally sturdy layers below and eventually destroying collagen and elastin. Collagen makes your skin firm and youthfully plump. Elastin gives skin resiliency and keeps expression lines from forming into creases and wrinkles.
I recommend sunscreens with at least SPF 30 that contain physical blockers such as zinc or titanium. A couple of my favorites are SkinMedica Daily Physical Defense SPF 30+ and SkinCeuticals Ultimate UV Defense SPF 30. EltaMD and Obagi also make great sunscreens. Don’t be afraid to use sunscreen around the eyes. If you become irritated, try a new sunscreen or ask your dermatologist for help and make sure you keep that area moisturized! Also, wear sunglasses to help avoid squinting.

SkinMedica TNS Eye RepairInvest in a good eye cream and moisturizer. If your eye cream is not moisturizing enough, make sure you moisturize over it. Many eye creams claim to erase dark circles and fine lines—while I think that many of these creams overpromise, I do think that long term use of an antioxidant, retinol and/or growth factor product (such as SkinMedica TNS Eye Repair or Lumiere Bio-restorative Eye Cream with PSP) in this area will definitely help. Antioxidants like green tea (found in Replenix products) or coffeeberry (found in Revaleskin products—both of which have great eye creams), work to fight free radicals, which are produced by pollution, stress and sun—a skincare nightmare! They attack healthy skin cells and cause collagen breakdown, which leads to wrinkles.

If you’re not using a retinoid cream, give us a call, because you should be. Retinoids include prescription-strength tretinoin (Retin-A), over-the-counter retinol and retinylaldehyde-containing products and should be incorporated into your skin care regimen indefinitely. Retinoid creams used nightly (or every other night), stimulate collagen renewal and prevent skin cells from breaking down. Women in their 20s and early 30s can use these to keep fine lines from turning into deeper creases. These products are often available by prescription or by direct dispense from a physician’s office. There are also some good over-the-counter options, such as SkinMedica Tri-Retinol Complex. For dark circles due to pigment, a hydroquinone-containing cream (such as EpiQuin in our SkinMedica Hyperpigmentation System) may be necessary. There are also some good non-hydroquinone lightening agents available today (products by DCL and NeoCutis are among our favorites).

Botox or Dysport injections are a great way to reduce existing lines due to movement, and also to prevent etching in of further lines down the road. In most individuals, crow’s feet are caused by smiling and squinting in combination with loss of facial volume and collagen. When it comes to the eyes, only a small amount of Botox can make a huge difference. Even treating the “bunny lines” can improve the appearance of the lines under the eyes.

Depending on the amount of volume loss, fillers such as Juvederm or Restylane may be injected in to correct some of the deeper lines. These fillers can also mask tired-looking “bags” and dark circles under the eyes. By filling in the hollow around the eye socket you can reduce shadows, puffiness and the sunken eye appearance.


Eyes Treated with EnerPeelChemical peels in the eye area are a great way improve the appearance of dark circles due to pigment, fine lines, and crepiness. Cathy’s Tips on Getting the Most from Your Products

Following these tips by our fantastic esthetician, Cathy, will no doubt result in glowing, radiant skin! 



Clarisonic Mia Sonic Skin Cleansing System

Think of the surface of your skin—those dead cells—as a barrier. You have to remove the barrier for products to get in. Basic cleansers are great for removing dirt and oil, but to help increase absorption of your products, it helps to use an alpha-hydroxy cleanser, such as Glytone Mild Gel Wash or Cream Wash to chemically exfoliate. Or you can mechanically exfoliate with a scrub containing small, smooth particles, like SkinCeuticals Micro-Exfoliating Scrub or by using a gentle buffing tool. I recommend the Clarisonic Mia Cleansing System for a deep clean. The Mia buffs your skin to a healthy glow, leaving your skin uber clean and allowing products to penetrate up to six times deeper.

Whatever goes on first penetrates best. So whether you’re fighting wrinkles, zits, or sun spots, the most active ingredients should be applied first. If you’re using two products for two different problems, apply one to freshly cleansed, bare skin in the morning and the other to bare skin at night. If you have multiple treatment products, layering is okay—just use the thinnest first. Smooth on other items in order of density, from thinnest (like an antioxidant serum) to thickest (moisturizer or night cream).

Damp skin acts like a sponge, quickly absorbing whatever comes its way. (Make sure skin is damp…not wet.) This rule applies to almost every product—especially products for the body. Increase absorption even more by washing your face with lukewarm water instead of cold. Warm water causes pores to dilate therefore letting more product in. The only caveat: if you have sensitive skin, you may need to let your skin fully dry!

Skinceuticals Micropeel: Dermaplane Blade + Solution

Dermaplaning is a gentle skin resurfacing procedure utilizing a special surgical blade to remove the top layer of dead skin cells. The removal not only allows for cell renewal, but also allows for better penetration of professional and home products, optimizing their results. As an added bonus, it also removes facial hair—a great procedure for patients wanting to remove lighter-colored facial hair or “peach fuzz,” which doesn’t respond to laser treatment. With absolutely no downtime and zero discomfort, this is one of my favorite treatments!

Serums are an important piece of the skin care puzzle and can help to transform your skin. Serums have the ability to penetrate more deeply into the layers of the skin where they deliver specific ingredients that have very explicit jobs. They are nice to layer under moisturizers. Looking for glowing skin? Try Obagi’s Professional-C Serum 15% to pump the skin full of antioxidants!

Obagi NuDerm SystemSunscreen and antioxidants were born to be married. As sunscreen blocks UV rays, antioxidants combat the free radicals, which leads to sun damage and wrinkles before they can do damage. If your SPF doesn’t include them, layer an antioxidant serum underneath. (Try SkinMedica Vitamin C+E Complex). Some other popular couples are retinoids and hydroquinone to treat hyperpigmentation and melasma (Obagi’s NuDerm System). They get out pigment in different ways and work best in conjunction. Knowing when to mix and match can be confusing, so feel free to check with us anytime!

I haven’t met anyone who doesn’t love February’s Product of the Month, the Clarisonic Mia Sonic Skin Cleansing System—it’s like an electric toothbrush for your face.

Treating + Preventing Razor Bumps

Art of Shaving Safety RazorThere are two causes of razor bumps: transfollicular penetration and extrafollicular penetration. If you have curly hair under the skin’s surface, it can grow straight into the sidewall of the follicle—that’s transfollicular penetration. Or, the hair grows out of the hair follicle and then curls and pokes back in—that’s called extrafollicular penetration. Either way, this causes an inflammatory reaction, or a razor bump.

Here are some tips to help treat and prevent those pesky bumps:

  • Use a sharp razor—change blade every 2-3 shaves.
  • Use a single blade razor. Too many blades cuts the hair too short.
  • Shave daily. Don’t give the hair a chance to grow too much.
  • Shave with the grain, don’t stretch the skin, and use light pressure.
  • Use a shaving gel or cream—you want a slick surface to prevent trauma.
  • Make sure the area is warm and hydrated. Don’t shave right when you get in the shower. Give yourself a few minutes. Dry hair that is cut has a sharp, pointed tip that is more likely to repenetrate the skin causing razor bumps.
  • For men who shave on a regular basis, use a soft-bristle toothbrush or Clarisonic at bedtime to gently massage (in a small circular motion) the face and neck. That helps to take out hairs that are starting to grow in, so they don’t continue growing throughout the night. But, be gentle.
  • If using an electric razor, use low setting.
  • Use an exfoliating product, like a lotion or cleanser with salicylic acid or glycolic acid.
  • Consider laser hair removal.
  • If you are still having trouble, make an appointment with a dermatologist. Some people need a prescription product.

Facial telangiectasia consist of small dilated blood vessels near the surface of the skin. They are harmless and can develop anywhere on the body but are commonly seen on the face around the nose, cheeks, and chin. They come with pregnancy and are common in patients with acne rosacea.

Cherry angiomas are also extremely common, come with age, and sometimes with pregnancy. They, too, are harmless clusters of capillaries at the surface of the skin—sometimes red, sometimes purple.

Treating them is quick and easy. We use a laser called a KTP—a dermatology workhorse. Just takes a few minutes, relatively painless, no downtime, usually just one session. You may have some redness and transient swelling, but it doesn’t last longer than a day.

Skin Care Resolutions for 2012

Manhattan Dermatology Skin Resolutions

  1. I resolve to incorporate an alpha-hydroxy acid product into my regimen, whether it be a cleanser like Glytone Mild Gel Wash or a cream like SkinMedica 15% AHA/BHA Cream, 2-3 times per week. This will help keep me glowing and reduce the appearance of fine lines, wrinkles and dark spots.
  2. I resolve to be diligent about sunscreen application, even on cloudy days, because ultraviolet rays, especially UVA rays are omnipresent. They even penetrate window glass.
  3. I resolve to get a skin cancer screening at my local dermatologist’s office. It’s time to check that spot. Melanomas have nearly a 100% cure rate if caught early.
  4. I resolve to stick to one skin care regimen for at least 3 months. With so many products on the market, people switch around too often. Invest in a good multi-step regimen (cleanser, treatment product, moisturizer, sunscreen) for your skin type and use it everyday, twice a day, for 3 months. I know you will see a difference in your skin. Be persistent and consistent!
  5. I resolve to make that favorite man in my life wear sunscreen everyday. Men tend to like sheer products—some nice ones to try are LaRoche Posay Anthelios 45 Ultra-Light Fluid for Face, or the chemical-free SkinCeuticals Sheer Physical UV Defense SPF 50. It will help prevent skin cancers and premature aging.
  6. I resolve to use one product that makes me smile. One of my favorites is Yonka Phyto Contour Eye Firming Cream, which has a great scent and has a refreshing, cooling effect. Or one of the many masques by Eminence—you can’t lose with a little alone time and one of these organic, amazing concoctions.
  7. I resolve to pamper my eyelashes with an eyelash growth product like Latisse. It works. Longer, darker, fuller lashes enhance any skin care routine. As we age, our lashes become thinner. It’s important to give them a boost. If you want to opt for a non-prescription option, you may want to try RevitaLash, or the one that I am using now, Jan Marini’s Marini Lash Eyelash Conditioner.
  8. I resolve to maximize my hair, nail, and skin health by eating better and incorporating supplements into my life, like Murad Firm & Tone Supplements or Perricone MD Superberry Powder with Açai. I will eat more fruits and vegetables, fish, nuts, eggs, healthy oils and whole grains.
  9. I resolve to moisturize. People often forget this simple step. Alone, it will make your skin look and feel better. My favorite basic moisturizer is CeraVe Moisturizing Lotion. It’s easy to find, has a great price point, and everyone can use it—babies, acne-prone kids, parents and grandparents. It’s great for both the face and the body. The best time to moisturize is right after you shower while the skin is still damp.
  10. I resolve to try a chemical peel or a facial. Go for it. You will love it. Just like exercise, it’s an important part of healthy skin maintenance. We have an amazing esthetician, Cathy. She will make your day!

Baths + Showers:
We all love long baths, but prolonged hot water can dry out your skin. Instead, try a warm 10-minute shower once daily and don’t scrub too vigorously.

Soaps + Cleansers:
Keep your cleansing to a minimum and use a mild soap (like Dove) or a gentle soapless cleanser such as CeraVe Foaming Facial Cleanser or Cetaphil Restoraderm.

Fall is a great time to think about visiting your dermatologist for a total body skin check. Did you know that you can develop skin cancers in your scalp, mouth, or other places where the sun doesn’t shine?

Annual skin checks should start in your 20’s, but you should come in anytime you have a non-healing, persistent lesion (like a sore that doesn’t heal) or a new or changing mole. Recently, one of my patients came in with the mole picture here on her leg. It turned out to be a severely atypical mole and needed to be removed. This is just the kind of thing you should be looking for.

New FDA Labeling Guidelines for Sunscreens

Did everyone see the new FDA labeling guidelines for sunscreen? The take-home message is to use a broad spectrum sunscreen with an SPF 15 or higher in order to decrease your risk of skin cancer. Some of the major points…

Broad Spectrum:

The FDA now requires sunscreen to pass a standard test in order to be labeled as “broad spectrum.” Previously, products could use that verbiage without meeting any requirements or standards. Broad spectrum means that a sunscreen protects against both UVA and UVB rays. UVB rays have long been known to be “burning” and “cancer causing” rays (SPF only takes into account UVB). Although UVA rays can contribute to skin cancer, they are generally considered more to be “aging” rays. They penetrate deeper than UVB rays, and are more damaging to collagen, leading to premature wrinkles. UVA rays can penetrate window glass (beware, commuters!), and are omnipresent at all hours of the day.

Use at least SPF 15:  

The FDA also acknowledges that sunscreens with an SPF of 15 or higher are needed to prevent skin cancer. So, even if the label says “broad spectrum,” and the SPF is only 8, it is not enough to protect you from skin cancer and you are at risk for burning. So why SPF 15? An SPF 15 will block about 93% of the sun’s UVB rays. An SPF 30 is better, capturing a little more protection, about 97%. By the way, the new guidelines have capped at SPF 50 which provides about 98% protection.

We live in a beach community—we need to enjoy the weather and the outdoors, but let’s do it right. Try not to get repeated sunburns and please don’t deliberately tan (tanning booths are off limits). When you’re spending time outdoors, bump up the SPF and reapply often, about every 2 hours, and always after swimming (the terms waterproof and sweatproof are no longer allowed on sunscreen labels). As long as you use enough—about a teaspoon for the face and neck, and a shotglass for the body—you should be getting adequate protection. Sunscreen becomes less elegant as the SPF increases, but if you can find one that you like with a higher SPF, please use it.

Shopping for sunscreen can be overwhelming. Some of my favorites are SkinCeuticals Physical UV Defense SPF 30SkinCeuticals Physical Fusion UV Defense SPF 50 (now with tint—a good matte option if you have oilier skin), Dermalogica Super Sensitive Faceblock SPF 30Nia24 Sun Damage Prevention SPF 30, and Glycolix Elite Sunscreen SPF 30. And for the kids, I like Blue Lizard Baby SPF 30.

Why won’t these bumps go away?

What are those pesky skin colored bumps on the back of your arms? You pick at them, but they don’t go away. It looks like you have chicken skin. You’ve tried countless creams and concoctions, but nothing seems to work. Maybe your mom or dad had them. Maybe your kids have them. Maybe they worsened with pregnancy or appeared after childbirth. Those troublesome bumps have a funny name–they are called keratosis pilaris.

Keratosis pilaris (or KP for short) is a common skin condition that tends to run in families and tends to affect people with sensitive skin or a history of eczema. They most commonly affect the back of the arms, but they can appear on the cheeks, thighs, and buttocks. Sometimes they are confused with acne. The bump is actually dead skin material (keratin) that can’t get out of the hair follicle because it’s “sticky,” a trait that is probably genetic.

Although we can’t cure KP, we certainly can try to improve the appearance and soften the bumps. Treatment consists of gentle manual exfoliation combined with chemical exfoliating agents like glycolic acid, lactic acid or urea-containing creams and lotions. I usually recommend Glytone’s Keratosis Pilaris Kit, which comes with an exfoliating wash, a loofah, and a rather potent moisturizer containing 17.5% glycolic acid, Glytone Retexturize Body Lotion). Daily use of the glycolic acid containing moisturizer can be irritating for some people if used everyday, so I often tell my patients to alternate the use of the Glytone Retexturize Body Lotion with a more gentle CeraVe Moisturizing Lotion or Moisturizing Cream.

Dry skin can make KP worse so it’s really important to maintain a good skin barrier with daily use of a moisturizer. CeraVe lotion or cream is a fabulous daily moisturizer as is Glytone’s less potent Retexturize Body Lotion SPF 15 (with 6.2% glycolic acid).

Sometimes the bumps can get red, itchy and angry—when this happens, you may benefit from a lotion containing a topical steroid, either over the counter or from your dermatologist. If you want to do more, you could choose to do microdermabrasion or a series of chemical peels. The most important thing—try not to pick! You may run the risk of leaving a blemish that may take months to go away, or even scar.

I had my first dermaplaning experience recently. Actually, thanks to our wonderful esthetician, Cathy Wainwright, our entire office staff is now free of “peach fuzz” and most of our stratum corneum (the top, dead layer of skin) after our dermaplane party. Our skin is now super soft and glowing. As a dermatologist, I was surprised that I wasn’t familiar with this extremely popular esthetician procedure, but I am so happy that I was introduced to this quick, easy, no downtime treat. Dermaplaning is a gentle skin resurfacing procedure utilizing a special surgical blade to remove the top layer of dead skin cells. The Skin Care Pillars

I’m a dermatologist—an expert in everything skin, hair and nails. So why do I have heart palpitations when I walk down the skin care aisle at CVS? Is it just me, or has there been an explosion of new beauty products lately? Often times it’s impossible to really know how well a certain product does what it says it will do and where it’ll fit within your skin care regimen. Cosmeceuticals (over the counter cosmetic skin care products) are not really regulated (they don’t need to be) and they don’t have to show us their efficacy studies or how well a certain ingredient in their formulation penetrates the skin, or how active their ingredients are.

Most dermatologists feel like there are certain categories of products that are important. These are the skin care pillars, if you will: Retinoids, Antioxidants, Growth Factors, Peptides, Alpha or Beta Hydroxy Acids (think glycolic or salicylic acid)—oh, and of course Sunscreen. You don’t have to use them all at once (but if you’d like to, you can). You can rotate them around or find your favorite two and stick with them until the next best product comes out that you want to try. You can use one in cleanser form, one in serum form, the other in moisturizer form. Confusing, right? I tell most of my patients to take at least these steps: cleanse, apply a treatment product (like an antioxidant, retinoid, or growth factor product), moisturize, sunscreen, makeup. Repeat this skin care regime at night, just skip the last two. I am going to go out on a limb and tell you some of the beauty products that I like (and that I use). They have good science behind them and some actually have published studies in dermatology literature.

Retinoids: If you can’t make it to your dermatologist to get a prescription strength retinoid or have “been there, done that” and feel like you can’t tolerate one of these formulations, you may want to try SkinMedica’s Tri-Retinol Complex or Tri-Retinol Complex ES. The easiest thing to do when shopping for a retinoid is to look for the word “retinol.”  Some are stronger than others, but you will still get some benefits even with the lower-concentration products. Retinol is everywhere, but some common lower-priced products are Neutrogena’s Healthy Skin, Roc Retinol, and Affirm creams. You can’t forget to moisturize when using these products, either on top, over it, or mixed with it. Use these products at night. Bottom line: retinol works. Give it a few months. Your skin will look better and more even-toned, and your fine lines should look less noticeable.

Antioxidants: My patients seem to love SkinCeuticals Phloretin CF. I use it myself to fight my battle with the freckles on my arms and hands every day—it’s thin, easy to spread and a little goes a long way. I also like SkinMedica’s Vitamin C+E Complex—it’s soft and velvety.

Vitamin C is confusing and hard to formulate—there’s an ongoing debate as to which form is the most stable, which form gets to where it needs to go to improve your skin, and which form actually works the best. I have spent a lot of time trying to figure this out. But, we do know when vitamin C works, it works. Period. It stimulates collagen, minimizing lines and wrinkles. It also lightens dark spots and protects you from the sun. Other brands that I think are good (not just for antioxidants) are PCA and DCL. Other good antioxidant products are Replenix Power of Three Serum or Cream and Replenix CF serum or CF cream, both of which have green tea, and RevaleSkin, which utilizes CoffeeBerry. We generally say to use these products in the morning to help prevent free radical damage you’ll encounter throughout the day.

Growth Factors: There really is no comparison. If you want to go there, you must go with SkinMedica’s TNS Essential Serum(or any of their TNS products—they have growth factors, just in lower concentrations). The science behind it is there. The best part about this anti-aging product is that it also has antioxidants, peptides, lightening agents all mixed in.

A well-respected dermatologist stands behind it. It’s pricey and I’m sorry, but it’s amazing.

Peptides:  Think copper peptide or pentapeptides. I have a little trouble with this one because I am just not so sure that the science is as compelling as some of the other things I mentioned. We do know that peptides increase collagen production, which improves elasticity and decreases fine lines and wrinkles. Olay Regenerist is probably the most popular pentapeptide product, and my patients really seem to like it. To that point, if you find a skin care product that you love and it is doing the job, stick to it (and let me know!).

Alpha & Beta Hydroxy Acids: This is an easy one to do because they don’t have to be expensive and really do a nice job of exfoliating your skin. Absolutely essential for helping to remove the layers of dead skin that build up as we get older. You know what I mean! Your skin looks dull, your make up doesn’t go on as well, or those crow’s feet look more noticeable than usual? It’s time for a little help. Glytone makes great glycolic acid-based products.

Other brands that come to mind are MD Forte and NeoStrata. Some brands are trending toward malic and mandelic acids, also alpha hydroxy acids. Salicylic acid is a good choice if you have acne prone skin. Peter Thomas Roth Max Complexion Correction Pads are a favorite and cleansers like Glytone Acne Cleanser or DDF Salicylic Wash 2% are good ones to try.

Sunscreen: A must! I wouldn’t be a dermatologist if I didn’t go there. Don’t undo all the money you spent and all the progress you made with your products by skipping sunscreen. There are SO many great ones out there now. You can’t use the excuse of it being sticky or too white anymore. Apply it every day to your face, neck, décolleté, forearms and hands. Just make it part of your daily routine. If you are getting burnt despite your sunscreen, buy another brand or use a thicker layer. And look for the following ingredients: avobenzone, mexoryl, titanium dioxide or zinc oxide (or a combination of them) in high concentrations. I have “sunscreens of the moment” so I rotate a lot. One tube shouldn’t last you very long, and if it does, you are not using it properly. At this moment, I am loving SkinCeuticals Ultimate UV Defense SPF 30 (it’s moisturizing on my dry skin) and their new tinted Physical Fusion UV Defense SPF 50, which gives a bit more of a matte finish (great for reapplication or for more oily skin).

But I also really like Epionce (paraben-free, I like all their products), La Roche-Posay (their sunscreens all have either mexoryl or stable avobenzone combined with antioxidant goodies), Glycolix Elite Sunscreen SPF 30 (with 17% micronized zinc oxide—wow!), and Replenix CF SPF 45 which has avobenzone, micronized zinc oxide and green tea. Blue Lizard is my favorite for kids or for a day at the beach. The list goes on and on but I should stop for now.

One category that may be important in the future is DNA-repairing enzyme products. Two options that I know of are Neova DNA Damage Control sunscreen and the DNA EGF Renewal line by Dr. Ronald Moy. I think this may be the start of something good. Stay tuned.

To recap: there are a ton of skin care products out there. Just know that you should be doing more than simply washing your face and putting on a moisturizer when you feel dry. Now if you’ll excuse me, my husband is trying to take my three-year-old daughter to the park without sunscreen. Somebody help me.

Anti-Aging Skin Regimen for MEN

Morning + Night:

  • Wash (or shave—try Murad’s Cleansing Shave) with your favorite cleanser twice a day.



  • After washing, apply the serum and eye cream, and apply your favorite nighttime moisturizer, such as Zirh’s Protect, which is lightweight and also provides environmental protection.


2-3 Times a Week:

  • If your skin can tolerate it, it would be nice to use a gentle exfoliating wash such as Anthony for Men Glycolic Facial Cleanser at least 2-3 times per week. If your skin does not become irritated, you can use it daily.

SPF 101

Use of a sunscreen with SPF 15 is OK for the person who truly is indoors all day and doesn’t get sun exposure. But, for most of us, who have sunny commutes and like to enjoy some outdoor time during the workday, I want people to find at least an SPF 30 that they actually enjoy to use.

Formulations are so good these days, it’s not hard to find an elegant cream or lotion that moisturizes, protects, and can be worn under makeup. If you are out all day on the weekends, bump it up, and reapply often (every 2-3 hours). And, don’t forget the neck and chest.

Sunscreen not only protects you from developing skin cancer, but helps reverse and prevent photoaging. The difference between a patient who wore sunscreen for 20 years versus one that didn’t is remarkable.

The question of vitamin D always comes up in this conversation. As dermatologists, we don’t want our patients to seek the sun just to get vitamin D. The current recommendation for an adult is an oral supplement of 600 IU per day. You can ask your primary physician (or dermatologist) to do a simple blood test to determine your vitamin D level and to provide you with a specific recommendation.

Dr. M’s Brand Review: SkinMedica

SkinMedica is probably my favorite physician-dispensed product line. It was founded by a well known and well respected dermatologist who brings science to his brand. I love recommending SkinMedica not only because the products are outstanding, but because they’re backed by substantial clinical studies demonstrating their efficacy. And the line is very complete, spanning everything from growth factors and antioxidants to peptides, retinol and sunscreen. They also offer a simple acne line as well as a new redness cream with niacinamide.

SkinMedica offers an excellent vitamin C +E serum, retinol creams, eye creams and sunscreens—but the TNS Essential Serum is probably their most exciting product (TNS stands for Tissue Nutrient Solution). The TNS products contain a physiologically balanced mixture of growth factors not found in other product lines. It aids in boosting collagen production and fighting the signs and symptoms of aging skin.

Dermal Repair Cream – Wow. This is such a luxurious cream and it’s a dream to put on. In addition to being luxurious, it offers an intelligent blend of ingredients, backed by good scientific research. This rich emollient cream is a great anti-aging product which contains retinyl palmitate, which encourages cell renewal and stimulates collagen production, plus vitamin E and a lipid soluble form of vitamin C, important for combating free radical damage and skin repair. This is one of my mom’s favorite products. It is more expensive, but if you want to treat yourself, give it a try. It would make a great gift too, in part because of its superb packaging. I would recommend using this cream at night after cleansing. You could use it alone, or layer it over your favorite nighttime repair or serum product. For example, it can be used over the TNS Essential Serum.

TNS Essential Serum – One of my favorites. I always come back to this in my rotation of skin care products. It is more expensive, but worth it. TNS Essential Serum is one of the most complete skin rejuvenation products on the market and contains an important component not found in other products: growth factors. In addition to growth factors, it contains peptides, antioxidants like vitamin C, vitamin E, CoQ10 and green tea extracts, skin brightening agents, and collagen and elastin stimulating agents. Because there is no retinol agent in this product , you can apply a thin layer of a retinol-containing product after TNS Essential Serum. If you don’t have a prescriptions strength retinoid, Skin Medica makes a good one. Then, moisturize and apply a sunscreen.

TNS Recovery Complex – An excellent product that dermatologists became familiar with after its use as a post-procedure (laser, chemical peels) product produced great results. A growth factor containing product, it improves the healing process post-procedure, and also functions as an outstanding anti-aging product. Actually, TNS Recovery Complex is one component of the TNS Essential Serum—so if you were going to purchase one for daily use, I would choose the Essential Serum, which will provide you with a more complete product.
