This Fall: PDT for Sun Damage

Omnilux Blue Light LEDFall is the time for PDT at Manhattan Dermatology. PDT (or photodynamic therapy) is a great way to rejuvenate your skin and treat sun damage from those hot summer days. It’s also great for acne and can help those with oily skin.

A solution called Levulan is applied to the skin for about an hour (don’t you have some emails to catch up on?). While you “incubate,” this solution targets the abnormal sun-damaged cells. Then you sit under a blue light for a few minutes. This combination leaves the skin renewed, refreshed, and bright—but keep in mind, you won’t look your best for a few days after your treatment. Strict sun avoidance is necessary after the procedure for 24 hours.

How Hairstylists Are Helping to Fight Cancer

You think of your hairdresser as saving you from bad hair days. But now, your favorite stylist just may save your life. More than 80 percent of the most common types of skin cancers — squamous cell carcinomas and basal cell carcinomas — occur on the scalp, neck and face. Your hairstylists get a close look at your scalp when you’re in the chair, and they can play a particularly important role in skin cancer detection.

Who knew? Most skin cancers occur on sun-exposed areas, like the head and neck. Skin cancers of the scalp are easy for patients to overlook, which can unfortunately delay their detection. Studies show that hairstylists have been increasingly spotting skin cancers that may otherwise have gone unnoticed. “Hair stylists are often the first line of defense against skin cancers,” said Robert Pariser President of the American Dermatology Association. What’s also helping in the fight against skin cancer is cosmetology schools across the country are teaching students how to identify suspicious spots on a client’s scalp.

Researchers with the Harvard School of Public health surveyed 200 different stylists and found as many as 40 percent now check their customers’ scalps for suspicious moles and lesions. However the Skin Cancer Foundation advises everyone to check their skin head-to-toe monthly. Tinea Versicolor

If you grew up around here you have probably seen this or have had this yourself. Tinea versicolor is a fairly common rash, primarily affecting young adults. It appears when a yeast that normally lives on the human skin transforms into its fungal form—it does this with high temperatures and humidity, but can also be associated with excessive sweating, oily skin and pregnancy. It is not dangerous and not contagious, but most people are bothered by its appearance.

Tinea versicolor is usually asymptomatic, but may itch a little. It can present in different ways—most people develop tan or brown oval to round patches, but sometimes they look pale or pink. It affects the upper trunk and shoulders most commonly, but can spread to the neck, arms and beyond.

It’s easy to treat with antifungal creams and many times, an antifungal shampoo applied to the body in the shower can be quite effective. The problem is that tinea versicolor likes to recur and in some people, a maintenance regimen may be necessary. You can try an over-the-counter dandruff shampoo or antifungal cream, but be sure to use it for a least 2 or 3 weeks. If that doesn’t work, make an appointment with your local dermatologist!

POTM for June: SkinMedica’s Rejuvenative Toner

That is the question. No, toner is not necessary, but if you enjoy it, by all means, add that step to your regimen. I am a fan of finding a regimen that works for you, so if toning makes you happy, stick with it! (I love multi-step regimens, but then again, I am a dermatologist.)

In general, toners are used after cleansing to remove excess oils and residual makeup, but they are also used to restore the pH balance of the skin and to prep for the skin for the steps that follow. Toners also can be a great way to gently and easily cleanse in the morning for the more low-maintenance type.

There are many kinds of toners on the market that do different things and it can be confusing. Here’s the lowdown:

  • Some have a nice mix of exfoliators to help remove dead skin cells and allow your other products to work more effectively. One of my personal favorites is our June Product of the Month: Rejuvenative Toner by SkinMedica. Not only does it smell absolutely wonderful, but it has 7% AHA (alpha-hydroxy acids) including lactic, glycolic, malic acids, green tea extract, vitamin A and salicylic acid.
  • Toners can be very useful for people with oily skin. You want to look for a product with salicylic acid, which can help wick away excess oil and calm acne breakouts—a good one to try is Glytone’s Acne Cleansing Toner.
  • Hydrating toners, on the other hand, are probably not necessary if you are using a good moisturizer, but they can be very refreshing and many have soothing properties. One of my favorites is Murad Hydrating Toner, which may be a good choice for someone in their 40s or 50s with drier skin due to hormonal changes.
  • A couple of years ago, I gave one of my good friends Murad’s Essential-C Toner—and of all the things I have given her through the years, she absolutely loves this and feels like it changed her skin like no other product. So, I guess for some, toners are a must!

Two Great Options for Acne

If you struggle with acne, please don’t despair—we have solutions. Two options we are really excited about are LED light therapy and photodynamic therapy (PDT). Here’s how they work…

LED Light Therapy

LED light utilizes blue light (415nm) to target P. acnes, the bacteria associated with acne. P. acnes is not bacteria that spreads from one person to another—it’s actually part of our normal skin flora and contributes to the inflammation associated with acne lesions.

Blue Light LED Therapy Facts:

  • FDA approved for acne.
  • Safe and effective for all skin types, even during summer months.

PDT (Photodynamic Therapy)

Omniluxe PDTAnother great adjunct to traditional therapies, photodynamic therapy (PDT) also utlilizes blue light to treat acne. Now more mainstream, PDT is being recognized as a very effective treatment for acne. It can be alternative for patients who don’t want to take oral antibiotics or Accutane.

PDT uses a solution called levulan to further activate P. acnes (the bacteria associated with acne) and also targets the sebaceous (oil) gland. Levulan is applied to the face one hour prior to the treatment (you can check email, surf the web, or watch a movie/show on an iPad). After the solution is rinsed, you sit under the blue light for a few minutes.

PDT Facts:

  • Strict sun avoidance for 24-48 hours is necessary following PDT.
  • May have some post-treatment redness, warmth, tingling.
  • A great option for chest and back acne.
  • A good treatment for the reduction of pores, oily skin, and overall skin rejuvenation.

We recommend 3 treatments—once a month for 3 months. The cost is $350 per treatment.

If you’re a parent, you have probably seen molluscum contagiosum. Most of our kids have had them at one time or another. Molluscum are shiny, umbilicated (having a little central dell) bumps that are common in kids. Often confused with warts, MC is caused by a virus that is usually passed from skin-to-skin contact. They’re not dangerous and if left alone, they may go away with time. However, we like to treat them so that they don’t increase in number or spread to others.

We usually treat molluscum using cantharidin, the extract of the blister beetle commonly referred to as “beetle juice”—kids love the name and they also love that it is painless. We drop a little bit on each lesion and hope to get a mild reaction (maybe even a blister). Over the next few days to weeks, the lesions may get a little irritated and hopefully fade away. We usually treat them at one month intervals until they are gone.

Organic Skin Care

Natural and organic skin care products are tough and can be confusing for consumers. Even though the label may say organic, you really need to look for the organic seal, which actually differs from a product that is 100% natural. There are strict guidelines for a product to truly meet “organic” criteria, and few product lines do this.

While it’s a wonderful idea in theory, I have a hard time finding a product or product line that meets natural or organic criteria that is as effective in the anti-aging arena as their non-organic or non-natural counterparts. I feel like if you are going to invest in skin care, USDA Certified Organic Sealyou should choose products that are going to give you the best results for your particular skin type—it’s hard to do with the limited amount of natural or organic products on the market today. Also, most of the natural or organic products contain a lot of essential oils and botanical ingredients, which can cause irritation and allergy. Just because a product is organic, doesn’t necessarily mean that it is better for your skin.

That being said, I think the Suki line, which is 100% natural, does a great job. The Éminence line is a popular certified organic line with a wide array of excellent products—we carry the Yam & Pumpkin Enzyme Peel and love it. However, if you really want something that is going to fight wrinkles, fight sun damage, and help you turn back the clock, I would stick with a line that uses the agents proven time and time again to be efficacious—even if they contain synthetic ingredients.

Basal Cell Carcinomas

Five Warning Signs of Basal Cell Carcinoma

Basal cell carcinomas (or BCCs) are the most common form of skin cancer and something that we see on a daily basis in our clinic. They usually present as a little pimple or sore that doesn’t seem to go away or a persistent scaly spot. We see a lot on the nose, but others areas of the face are commonly affected—as are the ears, neck and scalp. I have been seeing a lot on women’s chests lately (remember those days of frying our chests?). Men’s backs are also a common area. These skin cancers are not life threatening and most can be treated easily in the clinic but it’s important to catch them early. If let go, they can become locally aggressive or could very rarely spread to other parts of the body. Sun protection is the best way to prevent them.

The Skin Cancer Foundation has outlined the five warning signs of basal cell carcinomas:

  1. An open sore that bleeds, oozes, or crusts and remains open for a few weeks, only to heal up and then bleed again. A persistent, non­-healing sore is a very common sign of an early BCC.
  2. A reddish patch or irritated area, frequently occurring on the face, chest, shoulders, arms, or legs. Sometimes the patch crusts, and it may also itch. At other times, it persists with no noticeable discomfort.
  3. A shiny bump or nodule that is pearly or translucent and is often pink, red, or white. The bump can also be tan, black, or brown, especially in dark-haired people, and can be confused with a mole.
  4.  A pink growth with a slightly elevated rolled border and a crusted indentation in the center. As the growth slowly enlarges, tiny blood vessels may develop on the surface.
  5. A scar-like area that is white, yellow or waxy, and often has poorly defined borders; the skin itself appears shiny and taut. This warning sign may indicate the presence of an invasive BCC that is larger than it appears to be on the surface.

Eye Got You Covered!

Why am I getting fine lines & wrinkles around my eyes?

One of the most common questions we get! Fine lines and wrinkles around the eyes are particularly common in both men and women. The onset of wrinkles in this area can occur considerably sooner than elsewhere on the face. The increased frequency of movement around the eyes—due to smiling, squinting and other facial expressions—allows for expression lines to become etched-in more quickly than the rest of the face. That, and the fact that the skin around the eyes is very thin, eye wrinkles can appear at almost any age. They will also appear more quickly if a person has a substantial amount of sun damage (sun worshippers, beware!). While there is no surefire way to guarantee you will never wrinkle, there are steps you can take to help prevent (and treat) eye wrinkles.

How to approach lines around the eyes?

Like most treatments in dermatology, I believe a multi-faceted approach will allow for the best outcome. I recommend tackling lines around the eyes in several ways. (Dark circles are a whole other topic, but these tips help those too!)

1) TOPICAL REGIMEN (SUNSCREEN, EYE CREAM + MOISTURIZER, RETINOID):

SUNSCREEN
SkinMedica Daily Physical Defense SPF 30+It is important to be diligent about sunscreen use to prevent further damage. Sun damage is the leading cause of fine lines and premature aging. In fact, many experts agree that wrinkles are the direct cumulative result of decades of unprotected sun exposure, rather than aging itself. Ultraviolet (UV) rays penetrate the skin, invading the normally sturdy layers below and eventually destroying collagen and elastin. Collagen makes your skin firm and youthfully plump. Elastin gives skin resiliency and keeps expression lines from forming into creases and wrinkles.
I recommend sunscreens with at least SPF 30 that contain physical blockers such as zinc or titanium. A couple of my favorites are SkinMedica Daily Physical Defense SPF 30+ and SkinCeuticals Ultimate UV Defense SPF 30. EltaMD and Obagi also make great sunscreens. Don’t be afraid to use sunscreen around the eyes. If you become irritated, try a new sunscreen or ask your dermatologist for help and make sure you keep that area moisturized! Also, wear sunglasses to help avoid squinting.

EYE CREAM + MOISTURIZER
SkinMedica TNS Eye RepairInvest in a good eye cream and moisturizer. If your eye cream is not moisturizing enough, make sure you moisturize over it. Many eye creams claim to erase dark circles and fine lines—while I think that many of these creams overpromise, I do think that long term use of an antioxidant, retinol and/or growth factor product (such as SkinMedica TNS Eye Repair or Lumiere Bio-restorative Eye Cream with PSP) in this area will definitely help. Antioxidants like green tea (found in Replenix products) or coffeeberry (found in Revaleskin products—both of which have great eye creams), work to fight free radicals, which are produced by pollution, stress and sun—a skincare nightmare! They attack healthy skin cells and cause collagen breakdown, which leads to wrinkles.

If you’re not using a retinoid cream, give us a call, because you should be. Retinoids include prescription-strength tretinoin (Retin-A), over-the-counter retinol and retinylaldehyde-containing products and should be incorporated into your skin care regimen indefinitely. Retinoid creams used nightly (or every other night), stimulate collagen renewal and prevent skin cells from breaking down. Women in their 20s and early 30s can use these to keep fine lines from turning into deeper creases. These products are often available by prescription or by direct dispense from a physician’s office. There are also some good over-the-counter options, such as SkinMedica Tri-Retinol Complex. For dark circles due to pigment, a hydroquinone-containing cream (such as EpiQuin in our SkinMedica Hyperpigmentation System) may be necessary. There are also some good non-hydroquinone lightening agents available today (products by DCL and NeoCutis are among our favorites).

2) INJECTIONS:

Botox or Dysport injections are a great way to reduce existing lines due to movement, and also to prevent etching in of further lines down the road. In most individuals, crow’s feet are caused by smiling and squinting in combination with loss of facial volume and collagen. When it comes to the eyes, only a small amount of Botox can make a huge difference. Even treating the “bunny lines” can improve the appearance of the lines under the eyes.

Depending on the amount of volume loss, fillers such as Juvederm or Restylane may be injected in to correct some of the deeper lines. These fillers can also mask tired-looking “bags” and dark circles under the eyes. By filling in the hollow around the eye socket you can reduce shadows, puffiness and the sunken eye appearance.

3) CHEMICAL PEELS:

Eyes Treated with EnerPeelChemical peels in the eye area are a great way improve the appearance of dark circles due to pigment, fine lines, and crepiness. Cathy’s Tips on Getting the Most from Your Products

Following these tips by our fantastic esthetician, Cathy, will no doubt result in glowing, radiant skin! 

 

1.EXFOLIATE FIRST

Clarisonic Mia Sonic Skin Cleansing System

Think of the surface of your skin—those dead cells—as a barrier. You have to remove the barrier for products to get in. Basic cleansers are great for removing dirt and oil, but to help increase absorption of your products, it helps to use an alpha-hydroxy cleanser, such as Glytone Mild Gel Wash or Cream Wash to chemically exfoliate. Or you can mechanically exfoliate with a scrub containing small, smooth particles, like SkinCeuticals Micro-Exfoliating Scrub or by using a gentle buffing tool. I recommend the Clarisonic Mia Cleansing System for a deep clean. The Mia buffs your skin to a healthy glow, leaving your skin uber clean and allowing products to penetrate up to six times deeper.

2. GO IN ORDER
Whatever goes on first penetrates best. So whether you’re fighting wrinkles, zits, or sun spots, the most active ingredients should be applied first. If you’re using two products for two different problems, apply one to freshly cleansed, bare skin in the morning and the other to bare skin at night. If you have multiple treatment products, layering is okay—just use the thinnest first. Smooth on other items in order of density, from thinnest (like an antioxidant serum) to thickest (moisturizer or night cream).

3. APPLY TO JUST-WASHED SKIN
Damp skin acts like a sponge, quickly absorbing whatever comes its way. (Make sure skin is damp…not wet.) This rule applies to almost every product—especially products for the body. Increase absorption even more by washing your face with lukewarm water instead of cold. Warm water causes pores to dilate therefore letting more product in. The only caveat: if you have sensitive skin, you may need to let your skin fully dry!

4. DERMAPLANE
Dermaplaning is a gentle skin resurfacing procedure utilizing a special surgical blade to remove the top layer of dead skin cells. The removal not only allows for cell renewal, but also allows for better penetration of professional and home products, optimizing their results. As an added bonus, it also removes facial hair—a great procedure for patients wanting to remove lighter-colored facial hair or “peach fuzz,” which doesn’t respond to laser treatment. With absolutely no downtime and zero discomfort, this is one of my favorite treatments!

5. TRY SERUMS
Serums are an important piece of the skin care puzzle and can help to transform your skin. Serums have the ability to penetrate more deeply into the layers of the skin where they deliver specific ingredients that have very explicit jobs. They are nice to layer under moisturizers. 6. BE A MATCHMAKER
Obagi NuDerm SystemSunscreen and antioxidants were born to be married. As sunscreen blocks UV rays, antioxidants combat the free radicals, which leads to sun damage and wrinkles before they can do damage. If your SPF doesn’t include them, layer an antioxidant serum underneath. (Try SkinMedica Vitamin C+E Complex). Some other popular couples are retinoids and hydroquinone to treat hyperpigmentation and melasma (Obagi’s NuDerm System). They get out pigment in different ways and work best in conjunction. Knowing when to mix and match can be confusing, so feel free to check with us anytime!

7. GET REGULAR FACIALS

Treating + Preventing Razor Bumps

Art of Shaving Safety RazorThere are two causes of razor bumps: transfollicular penetration and extrafollicular penetration. If you have curly hair under the skin’s surface, it can grow straight into the sidewall of the follicle—that’s transfollicular penetration. Or, the hair grows out of the hair follicle and then curls and pokes back in—that’s called extrafollicular penetration. Either way, this causes an inflammatory reaction, or a razor bump.

Here are some tips to help treat and prevent those pesky bumps:

  • Use a sharp razor—change blade every 2-3 shaves.
  • Use a single blade razor. Too many blades cuts the hair too short.
  • Shave daily. Don’t give the hair a chance to grow too much.
  • Shave with the grain, don’t stretch the skin, and use light pressure.
  • Use a shaving gel or cream—you want a slick surface to prevent trauma.
  • Make sure the area is warm and hydrated. Don’t shave right when you get in the shower. Give yourself a few minutes. Dry hair that is cut has a sharp, pointed tip that is more likely to repenetrate the skin causing razor bumps.
  • For men who shave on a regular basis, use a soft-bristle toothbrush or Clarisonic at bedtime to gently massage (in a small circular motion) the face and neck. That helps to take out hairs that are starting to grow in, so they don’t continue growing throughout the night. But, be gentle.
  • If using an electric razor, use low setting.
  • Use an exfoliating product, like a lotion or cleanser with salicylic acid or glycolic acid.
  • Consider laser hair removal.
  • If you are still having trouble, make an appointment with a dermatologist. Some people need a prescription product.

Facial telangiectasia consist of small dilated blood vessels near the surface of the skin. They are harmless and can develop anywhere on the body but are commonly seen on the face around the nose, cheeks, and chin. They come with pregnancy and are common in patients with acne rosacea.

Cherry angiomas are also extremely common, come with age, and sometimes with pregnancy. They, too, are harmless clusters of capillaries at the surface of the skin—sometimes red, sometimes purple.

Treating them is quick and easy. We use a laser called a KTP—a dermatology workhorse. Just takes a few minutes, relatively painless, no downtime, usually just one session. You may have some redness and transient swelling, but it doesn’t last longer than a day.

Manhattan Dermatology Skin Resolutions

  1. I resolve to incorporate an alpha-hydroxy acid product into my regimen, whether it be a cleanser like Glytone Mild Gel Wash or a cream like SkinMedica 15% AHA/BHA Cream, 2-3 times per week. This will help keep me glowing and reduce the appearance of fine lines, wrinkles and dark spots.
  2. I resolve to be diligent about sunscreen application, even on cloudy days, because ultraviolet rays, especially UVA rays are omnipresent. They even penetrate window glass.
  3. I resolve to get a skin cancer screening at my local dermatologist’s office. It’s time to check that spot. Melanomas have nearly a 100% cure rate if caught early.
  4. I resolve to stick to one skin care regimen for at least 3 months. With so many products on the market, people switch around too often. Invest in a good multi-step regimen (cleanser, treatment product, moisturizer, sunscreen) for your skin type and use it everyday, twice a day, for 3 months. I know you will see a difference in your skin. Be persistent and consistent!
  5. I resolve to make that favorite man in my life wear sunscreen everyday. Men tend to like sheer products—some nice ones to try are LaRoche Posay Anthelios 45 Ultra-Light Fluid for Face, or the chemical-free SkinCeuticals Sheer Physical UV Defense SPF 50. It will help prevent skin cancers and premature aging.
  6. I resolve to use one product that makes me smile. One of my favorites is Yonka Phyto Contour Eye Firming Cream, which has a great scent and has a refreshing, cooling effect. Or one of the many masques by Eminence—you can’t lose with a little alone time and one of these organic, amazing concoctions.
  7. I resolve to pamper my eyelashes with an eyelash growth product like Latisse. It works. Longer, darker, fuller lashes enhance any skin care routine. As we age, our lashes become thinner. It’s important to give them a boost. If you want to opt for a non-prescription option, you may want to try RevitaLash, or the one that I am using now, Jan Marini’s Marini Lash Eyelash Conditioner.
  8. I resolve to maximize my hair, nail, and skin health by eating better and incorporating supplements into my life, like Murad Firm & Tone Supplements or Perricone MD Superberry Powder with Açai. I will eat more fruits and vegetables, fish, nuts, eggs, healthy oils and whole grains.
  9. I resolve to moisturize. People often forget this simple step. Alone, it will make your skin look and feel better. My favorite basic moisturizer is CeraVe Moisturizing Lotion. It’s easy to find, has a great price point, and everyone can use it—babies, acne-prone kids, parents and grandparents. It’s great for both the face and the body. The best time to moisturize is right after you shower while the skin is still damp.
  10. I resolve to try a chemical peel or a facial. Go for it. You will love it. Just like exercise, it’s an important part of healthy skin maintenance. We have an amazing esthetician, Cathy. She will make your day!

Winter Proof Your Skin

CeraVe Foaming Facial Cleanser

If Jack Frost has been nipping at your nose, leaving your skin dry and irritated, Dr. M has a few tips to help you winter proof your skin and keep you glowing!

Baths + Showers:
We all love long baths, but prolonged hot water can dry out your skin. Instead, try a warm 10-minute shower once daily and don’t scrub too vigorously.

Soaps + Cleansers:
Keep your cleansing to a minimum and use a mild soap (like Dove) or a gentle soapless cleanser such as CeraVe Foaming Facial Cleanser or Cetaphil Restoraderm.

Moisturizers:
Immediately following your shower, gently pat skin dry and moisturize with a supple lotion or cream. Some of my favorites include CeraVe Moisturizing Cream, Epionce Extreme Barrier Cream, Aveeno and Eucerin.

Humidifiers:
For extreme dryness, a germ-free humidifier or vaporizer in the bedroom can really help.

Hands:
Keep your hands soft by moisturizing after hand washing, wearing gloves outdoors, and don’t forget the rubber gloves when you’re cleaning up.

SPF:
Don’t forget to apply sunscreen every day—in all seasons, in all weather. Make it part of your daily routine. Your skin is particularly susceptible to sun damage in the snow, so apply early and often!
